Background Verification Form: Employee Journey

Once the Admin invites an employee, they will receive an email from SpringVerify ( info@springverify.com ). The Subject line of the email will be: IMPORTANT - Company Name: Background Check Request


The invitation email will consist of a text in blue which says: Start your Background Check Now, the employee will have to click on this button to open the BGV form.

Once the form is opened, the employee will have to click on Get started to start the process. After starting the process these are the steps that the employees usually have to follow: 


*Note - This can vary according to the check configuration of the group.
  1. Basic Details: It would be auto generated. Please check your details and if you have a UAN number, please provide that. If not, that step can be skipped. Click on Next.

  2. Contact Details: Your email address would be auto-fetched as well. 

    1. Alternate email: If you have Alternate email address , please provide that.

    2. Phone number: Would be auto populated as well. Please check if it is correct

    3. Alt phone number: If you would like to provide that, Please do so. 

    4. There would be a button to turn on Whatsapp notifications. If you would like to receive whatsapp notifications about your verification. Please toggle that on. 

  3. Identity: For most of the companies, one ID proof is Mandatory i.e. PAN 

    1. Click on Add ID proof - Add both the front and back images of the ID proof. If you have both the images in the single file, please click on the check box which says “I have images of both sides of my ID in a single file” . 

    2. ID number would be auto populated from the image updated. 

    3. Click on save and proceed further. 

  4. Employment: Please enter your latest employment details first. 

    1. Click on Add Employment

    2. Add company details: All the fields with asterisk are mandatory. 

      1. Company Name

      2. Employee ID

      3. Location 

      4. Choose the country

      5. Designation

      6. Duration : Add start date and end date

      7. If you still working in the given company - Click on the check box “I currently work here” 

      8. Reason for leaving if you have already left the company. 

    3. Click on Next. 

    4. Select the document type - It can be, Relieving letter, 3 months pay slip or Experience letter. 

    5. Add your employment proof and click on next. 
      Note: If you are a fresher, you do not have any previous employment, and on the BGV form you are asked to fill in the employment details, you can update a placeholder document and enter your current employer's name. Once we receive the form, we will process this according to your employer's requirements.

    1. Add HR & Reporting Manager information :

      1. Full name 

      2. Email address

      3. Phone number
        Note: While including contact details for your references is not strictly mandatory, it can significantly expedite the verification process. We strongly encourage you to provide this information for a smoother and faster experience.

  1. Education:

    Step 1: Please add your education details here which would be for Undergraduate degree and Postgraduate degree. 

    1. Undergraduate degree: Click on Add education 

    2. Add College Name

    3. University/Board

    4. Attendance Type

    5. Registration number - The registration/roll number refers to the number that is assigned to you upon enrollment in a school or educational institution. You can find these details either in your mark sheet or certificate.

    6. Degree: Eg: Bachelor of Technology or Master of Design

    7. Area Of Study: Eg: Electrical Engineering or Computer Science

    8. Dates Attended : From and To or Expected Graduation Date. 

    9. Add all these details and click on NEXT. 

    10. Step 2: Add your Degree Certificate and add All your marks sheets. And click on save. 

    11. Kindly follow the same steps of a Postgraduate degree too.

  2. Address :

    Step 1: Add your both permanent and current address details here. Click on Add Address under Permanent Address. Please provide the below details:

    1. Address of your permanent address: Flat/House number, Floor number and building number or name if it is an apartment. 

    2. Colony, street name 

    3. Landmark

    4. Pincode

    5. Select the country

    6. City

    7. State

  3. Step 2: Please check if the map is pointing the right place, if not adjust accordingly. 

    1. How long have you stayed here - Mention for how long you have lived in this address by selecting from the dropdown.

    2. Click on the checkbox if your permanent address and current address are the same.

    3. Answer if you will be available in this address between 9AM to 5PM for verification. Click on Yes or No. 

  4. Step 3: Select the Document type for verification. 

    1. Add the front and back of the documents. IF both the sides of the image are in a single file, please select the check box. 

  5. Consent form: (Company)  has provided us with your signed Consent Form for running the background verification. You can go ahead and submit your details now. 

    1. Please click on the Consent Form in blue to check all the details and click on Submit. 

  6. By clicking on the Submit button, you would have submitted your BGV form. If you want to make any changes before submitting, you can click on the Go Back button and make any changes. Once you have clicked on the Submit button, no changes can be made.
